We at Elder Resources Unlimited understand the problems you face, and are here to help. We provide a multitude of services including but not limited to: representation and coordination of  doctor’s appointments, oversight during  hospitalizations, short and long term placement in nursing, assisted and independent living facilities, basic bookkeeping, medication monitoring, assistance with applications to subsidized programs, transportation arrangements, coordination of homecare and the monitoring of caregivers. We take great pride in devising an individual care plan for each client that promotes their independence and well being, providing them and their family members with peace of mind.

Care Management services can be key in decreasing the amount of times a person is hospitalized. By seeing the client on a regular basis things that may have been missed or pushed aside will be addressed and taken care of immediately. Coordination of doctors appointments alone can be more then overwhelming especially if the individual is having some memory difficulties. Repeating tests and lab work is almost inevitable since most seniors have multiple physicians. Care Managers can provide the information that the doctors need and that are often overlooked by the client which can decrease medical costs as well as provide the person with less trips to the doctor. The ability of a care manager to be able to advocate for the client while promoting their self determination is key.

Julia J. Knoess, BSW, CMC started Elder Resources Unlimited, Inc. in 1999. After working in and acute care hospital for ten years as a medical social worker/ case manager, Julie realized the importance of continued care and monitoring once an individual was discharged from the hospital thus promoting optimal health. With over twenty-two years experience in the field of geriatrics she is well accustomed to the problems our seniors face in this day in age. She is certified in Care Management, is a member of the National Association of Professional Geriatric Care Managers, has a Bachelors Degree in Social Work from the University of Tampa and is a native Floridian. She has extensive experience in crisis intervention and has expertise in all areas of community resources including Medicare, Medicaid, and HMO eligibility. The dedication and care she provides to her clients and there families is unsurpassed.

What is a Geriatric Care Management?

Geriatric Care Management is the professional support and assistance to seniors and their families in the delivery of health care services, household assistance, facility placement, health care referrals and in-home services.

The goal is to maximize the levels of service provided seniors, fulfill unmet needs, and provide a link of communication to loved ones.

Does my insurance policy pay for these services?

Medicare does not cover the cost of Care management services. Some long term care policies do provide for services with a cap on the benefit however each individuals policy is different as are the parameters of reimbursement.

How are services billed?

-The client  is billed for an initial assessment at a flat rate
-Ongoing care management services are then billed at an hourly rate.
